Make Your Own DNA Kit
The following steps outline the DNA collection process for the “Make Your Own” DNA Kit. For each person tested, you will take an oral swab from the inner cheek and seal inside a paper envelope. Samples will be combined and mailed to the laboratory.
Estimated Completion Time: 5-10 minutes per person.
Step 1. Swab the inner cheek
This step collects a DNA sample from one person.
(1) One double-sided cotton swab per tested person or (2) Two single-sided cotton swabs per tested person
1.Swab the inside of the cheek with medium pressure for twenty (20) seconds.
2.Repeat using the other end of the cotton swab or another single-sided cotton swab.
3.NOTE: You are collecting skin cells, not just saliva. Use enough pressure to ensure you gain enough cells without causing harm.
Step 2. Seal inside a paper envelope
This step separates the individual DNA samples.
(1) One standard paper envelope
1.Place one person’s cotton swab(s) into a standard paper envelope.
2.Seal the envelope with tape. Do Not Lick the Envelope.
3.NOTE: Do not use plastic bags or lick the envelope. Doing so may contaminate the DNA sample.
Step 3. Write the person’s details on the envelope
This step provides information on the collected DNA sample.
Please write the following information on each DNA envelope:
1.Order ID: Found in your confirmation email.
2.Name: First and Last name of the collected DNA sample.
5.Date of Birth
6.Role: Such as (Father, Child, Mother, Grandparent, Aunt, Uncle, Sibling, Other)
7.Race: Such as (Asian, African American, Caucasian, Hispanic, Native American, Other)
8.Signature: From the person or a legal guardian
Repeat steps 1-3 for each person tested.
Step 4. Combine and Send the DNA samples
This step concludes the “Make Your Own” DNA Kit process.
(1) One mailing envelope with postage
1.Place all sample envelopes into a mailing envelope.
2.Use the following address on the mailing envelope:

By collecting and submitting a DNA sample, each party agrees to the following terms and conditions:
I do certify that I have the legal right to consent to have the enclosed DNA sample(s) tested for the purposes of determining a biological relationship. I understand that having self collected the samples within the envelope that there is no established chain of custody. With my signature, I consent to have this sample tested and I verify that the submitted information is correct.
